Don't mean to hijack the thread by adding a new issue, but I think
this one is related to these app icons breaking. I think that the app
icons are messing up because the apps themselves are reverting back to
old versions on a reboot. I've confirmed nearly 20 apps on the HTC
Hero that are reverting to old versions once you reboot the phone.
Here's the process:
1. Uninstall the old app, either through the market, through Manage
Applications, or through Safe Mode --> Manage Applications
2. Install the new version of the app, and start it up to confirm the
new version (Pandora is a good example as it shows app version in the
splash screen)
3. Reboot the phone
4. Any newly installed apps will revert back to their initial
versions. For example, Pandora will revert from 1.1.1 to 1.0.0, or
the first version you installed. Also reverting will be the initial
icons.
Here's a thread over at the XDA-Developers forum outlining the issue
in further detail if you wish to investigate it further:

Thanks for looking at this issue! Also, I'd be interested to know
HTC's bug reporting site too. They do seem that they like to not take
responsibility though, as they have pushed their problems with the
Daylight Savings Time messing up their calendar app to Google.
